The Idaho Home Outreach Program for Empowerment (ID-HOPE) provides Critical Time Intervention (CTI) services. CTI is a time-limited intervention lasting 9 months; it is divided into 3 specific phases and focuses on a limited number of treatment areas. The CTI team works with participants to create a recovery plan and build and strengthen linkages to both formal and informal supports in the community during the first phase. The CTI team works with participants to evaluate and adjust community resources as needed, assess and build upon strengths, and reinforce community linkages during the second phase. Participants continue to manage their formal and informal supports with support from the CTI team while transferring care to community supports during the third phase.

Provides family medical care, obstetrics, well child care, and pediatrics. Average first visit $150.00 PRENATAL CARE: Low-income persons without Medicaid or insurance and needing OB for pregnancy & delivery should be directed to the Business Office at (514-2500). Arrangements can be made for monthly payment plans which can be as low as $200 for the 1st visit and $200 a month to pay for delivery. Health Access - Provides primary care services to low-income, uninsured people in Ada County. They provide basic, physician office-type care including ongoing prevention, early detection, and chronic condition management. They will also provide basic medically necessary x-ray and laboratory services. To be eligible you must be uninsured (Medicaid and Medicare are considered insurance). If you miss two appointments in a six month period without advance notice, you would no longer be eligible for this program. Applicants must provide: Valid driver's license or proof of ID Address verification via utility bill Income verification HIV/AIDS - RYAN WHITE TITLE III CLINIC - PRIMARY MEDICAL CARE(SLIDING FEE) We offer care to HIV/AIDS patients. We also do free HIV testing. We help HIV/AIDS patients with housing etc. *** None turned away based on ability to pay *** WOMEN'S HEALTH CHECK contractor. Women's Health Check is a federally funded program that offers free breast and cervical cancer screenings to women who qualify. The Women's Health Check is for women who: do not have health insurance that covers mammograms or pap test, meet income guidelines, and are one of the following: Age 50-64 or; Age 30-49 and have not had a pap test in 5 years or longer, or never had a pap test or; under age 50 with symptoms suspicious of breast cancer (must have a clinician complete approval form) or; age 65 or older and is NOT eligible for Medicare, or does not have Medicare part B.

ASSISTED LIVING
The Arc assists in determining strengths, needs, job interests, and job matches for a variety of careers in employment. Individuals may select: Interviewing Skills, Job Seeking and Keeping Skills, Community Based Work Evaluations, Community Based Work Adjustment, and Personal Computer Training. Qualified Arc staff assist in job development, placement, job training, and follow up services.


VOCATIONAL ASSESSMENT AND JOB TRAINING
Work and Business Services
Production-based work includes: printing mailing, packaging, assembly, and paper contracts. Work can be completed at our main site or we can bring a supervised work group of to your site. Labor costs, liability insurance, employee benefits, and Worker’s Comp coverage are provided by The Arc. Work is secured through a competitive bidding process. One hundred percent quality control is guaranteed. Prevocational activities and work training are provided for persons with disabilities preparing to enter the competitive labor market.

ONE ON ONE JOB COACHING
Assistance is provided to individuals with disabilities to search for a job, obtain a job and learn the job with support from staff.

Recreation and leisure opportunities enhance each person’s quality of life, including sports, travel, dances, and special events. Vacation packages offer small, integrated travel opportunities at affordable rates. Services Provided: •PCIT was ranked 1 of 3 "best practices" in treating children who have suffered from physical abuse. The most appropriate referrals are children between the ages of 2.5-7 years who are exhibiting some challenging behaviors. Community Based Rehabilitation Services (CBRS)
This program is an individualized, community based service for adults and children with behavioral or emotional issues. We train our staff to utilize a multitude of approaches which allows them to effectively and efficiently support participants in developing skills to manage their symptoms and succeed in all areas of their lives, including work, school, family, and their community. CBRS assists people in developing appropriate skills for living independently and maintaining a healthy life. The training and certification our staff receives enables them to assist participants in successfully graduating from CBRS and integrate independently into the community. Staff certifications range from USPRA certification to Licensed Social Workers and Counselors.

The Benewah Medical Clinic has nurses that visit patients with diabetes, and educates them on diabetes and blood glucose monitoring. The clinic also has a glucometer program that provides glucometer training and education on what blood sugar means. People with diabetes are also educated on foot and eye care. They also have access to documents in Braille and large print documents.

Parenting classes which include the Positive Parenting model and Parenting as Prevention model. Class structure provides informal parent support group. On site child care provided.

The Safe Place locations have trained individuals who participate in the Safe Place program. They will assess the situation and make the appropriate referral or contact. Family Counseling Program provides crises intervention, individual and family counseling, mediation services and competency building on an outpatient basis to at risk youth and their families. Program includes the following components: Child Custody & Family Mediation; Victim Assistance Counseling; Victim/Offender Mediation; Testing & Assessment; Social Skills/Anger Management; Competency Building.

Out of Home Placement Program provides 8 out of home placements for young people in the custody of the State under Child Protection or Childrens Mental Health. Out of care is provided in the following two settings:
**Transitional Group Care Project - structured group care for adolescents in the custody of the State of Idaho.
**Treatment Family Program - a professional foster care program providing community based and family centered treatment to young people in State custody.

Bannock House provides emergency shelter, counseling and aftercare services to runaway & homeless youth and their families.

Runaway Homeless Youth Project provides emergency shelter only for adolescents. Counseling and aftercare services to runaway and homeless. They can house 13 youths, co-ed from ages 10-1.

Programs are administered by the Bannock Youth Foundation, a private, non-profit organization that has been providing a variety of intervention, treatment and support to youth and families in Bannock County since 1978.

Services Provided: The ISU Psychology Clinic is a training clinic for doctoral level students who provide psychological services under the supervision of faculty. Diplomate in Psychiatry, ABPN Dr. Ravsten is a native of Southeast Idaho, graduated from Idaho State University, attended the University of Osteopathic Medicine and Health Sciences in Des Moines, Iowa before completion of his Psychiatry residency at the University of Utah. He is a Board Certified Psychiatrist with a broad range of practice experience including work in outpatient and inpatient mental health settings. Board Certified Physician Assistant Kebai Bills, has joined Life Change Associates, PLLC with the goal of serving the psychiatric field by following her profession passion: to erase the stigma associated with mental illness and simply help people enjoy life to its fullest. She obtained her education through the physician assistant program at Idaho State University and completed her internship rotations throughout Idaho and Washington. With fond memories of growing up in the state, Kebai has remained in Southeast Idaho for the past several years, treating individuals with mental illness in settings ranging from community mental health centers to psychiatric hospitals. She has also taken part in forensic psychiatry in Bannock, Bingham and Power Counties.

Marco became a motivational speaker at the age of 19 combining his musical talents with the message of substance abuse prevention for adolescents. He realized after a brief time that it was necessary to further his education to make the greatest impact for helping those with substance abuse and mental health disorders. He attended Northwest College, Montana State University Billings, and finally received his Master’s degree in Psychology from Walden University. Marco is a Parenting with Love and Logic Facilitator with a strong background in substance abuse prevention programs, youth facilitation, and has been working with both children and adults mental health disorders in Idaho for over 10 years.
