Healthy Connections (HC) is a Medicaid program to assist individuals in obtaining a primary health care provider. The local HC Representative helps Medicaid recipients choose a doctor, or clinic that will provide their health care and provide referrals to other health care services as needed. Healthy Connections is administered by the Division of Medicaid.
