To Request a Public Record:
...The request must be in writing. Must include the requester's name, address and telephone number in the request.
...The request must be very specific, including dates, locations or names of reports if they apply to your request.
...If the request is regarding a family member or client, you must include a release signed by that person.
...If you are requesting client-specific information, complete the Records Request form located at www.healthandwelfare.idaho.gov, select Public Records (on the right side of the page) scroll to: To Request a Public Record and click on form in blue. If you submit the completed Records Request form to a local Health and Welfare office you will need to bring your identification. Your signature must be notarized if you submit the request form by mail, fax, or e-mail to PublicRecordsRequest@dhw.idaho.gov
