Utah Children's Care Coordination Network (UCCCN) - Update

The Utah Children’s Care Coordination Network (UCCCN) is a project of the Utah Bureau of Children with Special Health Care Needs (UDHHS) and the Medical Home Portal, intended to provide care coordination-related information, education, tools, support, and networking opportunities to pediatric practices and other health organizations focusing on children in Utah.
Current participants include those in care coordination roles from more than 23 practices, plus some health departments and health systems in and around Utah. Meetings focus on key topics recognized as components of quality care coordination in the Medical Home model, and include the opportunity to brainstorm about cases and to share local resources with each other.
We're looking forward in the coming months to presentations from:
  • The Utah Office of Home Visiting
  • Shriners Hospital - Salt Lake City
  • Utah Department of Health - Asthma Program
and meeting topics that will include:
  • funding care coordination in the practice
  • mental and behavioral health options for children in Utah
  • population management and health registries
If you have a care coordinator or employees who could benefit by attending these free meetings, have them contact us to join! Meetings are held on the 3rd Wednesday of each month from 8:30 – 10 am. Locations rotate between the Eccles Outpatient Building on the U of U Health Sciences campus or the IMC Doty Education Center in Murray. Our next meeting will be Wednesday, July 15th, at the Doty Education Center, 5121 Cottonwood Street. Customizable Local Service Provider Lists

A major project of the Portal's for some time now, we're excited to let you know that our beta feature is available to try on our Development server. You'll be able to maintain lists of regularly referred-to services for your practice that stay up-to-date through regular updates by the Medical Home Portal.

This feature allows you, after logging in, to quickly and easily create a custom list of local service providers in our database. You can then:
  • name it
  • share it via a link
  • make a copy to modify
  • save it as a PDF
  • print it

New and Updated on the Portal

We're very pleased about the publication of our new Premature Infant Follow-Up diagnosis module. Pediatricians have access to lots of information about how to care for preterm newborns in the NICU, but what about once they leave the hospital? The new Premature Infant Follow-Up module in the Medical Home Portal provides clinical information and guidance for the multiple conditions that can be associated with preterm birth. From immediate nutritional needs and tracking of growth to management of developmental issues and potential health problems, the module discusses these related issues and provides resources and services relevant to caring for preterm babies in the Medical Home.
